This is for a family that have not used Houghton Nursery Group before. | Additional Child Registration Form This is for a child/children whose sibling has attended (or attends) Houghton Nursery Group. | Change of Personal Details Form This form is to provide any changes to personal information, such as your address or phone number.

The two registration forms include a childcare agreement at the end that is required to be signed and returned to us. If the childcare agreement is not signed and you use our services, we will assume you accept the terms automatically.
